Popōca was a human who succumbed to the curse of the Thorned Emerald. In the first game, when McCoy is cursed like him, he was revived as a ghost spirit and tells him to bring the emerald to its home.
In Cactus McCoy 2: The Ruins of Calavera, however, he has a much more significant role, like coming out of every campfire checkpoint and restoring McCoy's health in specific points of the areas, like the beginning of a new section or near a boss fight.
He is McCoy's spirit guide and can appear wherever smoke rises.
